Job summary

Creative Solutions in Healthcare - East seeks a Speech Language Pathologist to join our in-house rehab team serving residents in Fort Worth, TX. The role offers full-time or PRN scheduling and focuses on swallowing and cognitive therapy to improve residents' independence.

Qualifications

• Active Texas license in Speech-Language Pathology.
• In good standing with regulatory agencies.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents' swallowing or cognitive deficits.
• Develop and implement individualized treatment plans to improve independence.
• Provide skilled, medically necessary speech and language therapy assessments.
• Provide AAC systems for individuals with expressive/language disorders.
• Educate residents and caregivers as a subject matter expert.
• Coordinate with other professionals for diagnostic swallowing procedures.
• Demonstrate competent technical skills according to standards of practice.
• Communicate changes in resident’s condition.
• Maintain accurate billing records and clinical documentation.
• Respond to family requests and concerns timely.
• Coordinate and provide training with Restorative Nursing and other staff.
