Job summary

COUNTRY OAKS CARE CENTER in California is seeking a Speech-Language Pathologist (SLP) to provide, direct and supervise all aspects of patient care in a post-acute setting. The role focuses on screening, evaluation, treatment planning and family education, ensuring care aligns with guidelines and standards.

CFY candidates will be considered. The SLP will coordinate with interdisciplinary teams and participate in quality assurance activities to support optimal patient outcomes.

Qualifications

• Master of Science in Speech/Language Pathology required.
• Licensure or Certification as required by State guidelines.
• CFY (Clinical Fellowship Year) considered for applicants.

Responsibilities

• Provide, direct and supervise all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Ensure patient care follows the organization’s guidelines, professional standards and community needs, and complies with Federal and State guidelines.
• Coordinate cross-disciplinary care and participate in quality assurance and customer service activities.

Provides, directs and supervises all aspects of quality patient care, including, but not limited to; screening, evaluation and treatment, treatment planning, goal setting, family education and documentation, in order to assure the best outcome possible for the patient. The Speech Language Pathologist (SLP) assures that the patient care follows the organization’s guidelines; professional standards and community needs, and is in compliance with Federal and State guidelines.

The SLP serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Ensures cross-disciplinary coordination in order to provide for consistent patient care. Participates in quality assurance and customer service activities.

Requirements

• Master of Science in Speech/Language Pathology.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• Students in their CFY (Clinical Fellowship Year) will be considered.
