Job summary

Heritage Manor Care in Idaho is seeking a licensed Speech-Language Pathologist to join our post-acute care team. You will provide comprehensive SLP services, screen and evaluate patients, develop treatment plans, and document progress in alignment with state guidelines.

Qualifications

• Master of Science in Speech/Language Pathology.
• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• Students in their CFY (Clinical Fellowship Year) will be considered.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation, in order to assure the best outcome possible for the patient. The Speech Language Pathologist (SLP) ensures that patient care follows the organization’s guidelines, professional standards and community needs, and is in compliance with Federal and State guidelines.
• Serves as a patient and rehabilitation advocate for current and prospective patients and for internal and external customers. Ensures cross-disciplinary coordination to provide for consistent patient care. Participates in quality assurance and customer service activities.
