Job summary

OneTeam Healthcare in Petaluma, CA is seeking a Speech-Language Pathologist (SLP) to join a supportive therapy team in a Skilled Nursing Facility. Qualifications

• Master’s degree in Speech-Language Pathology required.
• ASHA CCC-SLP preferred.
• Geriatric or Skilled Nursing Facility experience preferred, but qualified candidates from other settings are encouraged to apply.
• Strong communication and interdisciplinary teamwork skills.

Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders.
• Develop individualized treatment plans based on patient needs and goals.
• Provide dysphagia assessment and treatment.
• Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary team.
• Educate patients, families, and caregivers regarding treatment and strategies.
• Complete accurate and timely clinical documentation.
• Participate in care planning and discharge recommendations.
