OneTeam Healthcare is seeking a Speech-Language Pathologist (SLP) in Manteca, CA, to join a supportive therapy team in a Skilled Nursing Facility. Flexible full-time, part-time, or PRN schedules allow you to tailor your work hours while serving adult and geriatric patients.

The role involves evaluating and treating speech, language, cognitive-communication, and swallowing disorders, collaborating with an interdisciplinary team, and educating patients and caregivers.

Qualifications

• Master's degree in Speech-Language Pathology required.
• ASHA CCC-SLP certification preferred.
• Experience in SNF/geriatric settings preferred.

Responsibilities

• Evaluate and treat adults with speech, language, cognitive-communication, and swallowing disorders.
• Develop individualized treatment plans based on patient needs and goals.
• Provide dysphagia assessment and therapy.
• Collaborate with interdisciplinary team (PT, OT, nursing, physicians).
• Educate patients and families; document progress accurately.
• Participate in care planning and discharge planning.

Speech-Language Pathologist (SLP) – Skilled Nursing Facility

Schedule: Full-Time, Part-Time, or PRN

We are seeking a Speech-Language Pathologist (SLP) to join a supportive therapy team in a Skilled Nursing Facility in Manteca, CA. Responsibilities

• Evaluate and treat patients with speech, language, cognitive-communication, and swallowing disorders
• Develop individualized treatment plans based on patient needs and goals
• Provide dysphagia assessment and treatment
• Collaborate with PT, OT, nursing, physicians, and other members of the interdisciplinary team
• Educate patients, families, and caregivers regarding treatment and strategies
• Complete accurate and timely clinical documentation
• Participate in care planning and discharge recommendations

Qualifications

• Master’s degree in Speech-Language Pathology
• ASHA CCC-SLP preferred
• Skilled Nursing, rehab, or geriatric experience preferred, but qualified candidates from other settings are encouraged to apply
• Strong communication and interdisciplinary teamwork skills
