Job summary

Best Home Health Providers, Inc. is seeking a Speech-Language Pathologist to work in patients' homes, improving communication, function, and quality of life. You’ll collaborate with a supportive clinical team delivering coordinated care.

The role is per diem with a rate of $150 per visit, offering flexible scheduling, and opportunities to convert to full-time based on performance. Requires CA SLP license, CPR, and 1+ year experience.

Qualifications

• Master’s or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
