Job summary

Best Home Health Providers Inc. is seeking a Speech-Language Pathologist to provide in-home therapy across the Bay Area and beyond.

You will evaluate patients, create personalized care plans, and deliver therapy using specialized techniques in patients’ homes, collaborating with physicians and the clinical team. The role emphasizes patient-centered care, education for families, and ongoing professional development.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• CPR certification with AHA.
• Valid driver's license with insured, reliable vehicle.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.

As a Speech Language Pathologist with Best Home Health Providers, you will be working directly with patients and their families in their homes to improve communication, function, and quality of life. Please note that this role is per diem with a rate of $150 per visit.
