Job summary

Caring Hand Home Health Solutions, Inc. in Stockton, CA is seeking a Speech-Language Pathologist to deliver in-home therapy, improve communication, and enhance quality of life for patients and families.

You will collaborate with a clinical team to provide patient-centered care in home settings, with a focus on education, safety, and outcome-driven practice. The ideal candidate holds a Master’s or doctoral degree in Speech-Language Pathology, an active California SLP license, and at least 1 year

• Minimum 1 year of clinical SLP experience preferred.

Responsibilities

• Evaluate patients and develop individualized care plans for speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment; recommend communication devices as needed.
• Collaborate with referring physicians and care team for integrated treatment.
• Educate patients and families on treatment plans and goals.
• Maintain accurate clinical documentation per policy.

As a Speech Language Pathologist with Caring Hand Home Health Solutions, you will be working directly with patients and their families in their homes to improve communication, function, and quality of life. Please note that this role is per diem with a rate of $150 per visit.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.
• Valid driver's license and insured, reliable vehicle.
• Strong communication and organizational skills.
