Job summary

Best Home Health Providers is seeking a compassionate Speech-Language Pathologist to deliver in-home services. You will evaluate, plan, and implement therapy for speech, language, cognitive, communication, and swallowing disorders as part of an interdisciplinary team.

The ideal candidate holds a Master's or Doctoral degree in SLP, a current state license, and at least one year of clinical experience. Strong communication, organization, and CPR certification are required.

Qualifications

  • Master's or Doctoral degree in Speech-Language Pathology.
  • Current Speech-Language Pathologist license in the state where services are provided.
  • Minimum of one year of clinical experience in Speech-Language Pathology preferred.
  • Strong verbal and written communication skills.
  • Excellent organizational and time management skills.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and current auto insurance.

Responsibilities

  • Conduct comprehensive speech-language evaluations and assessments.
  • Develop individualized treatment plans to address speech, language, cognitive, communication, and swallowing disorders.
  • Implement therapeutic interventions using appropriate techniques and equipment.
  • Recommend communication devices and adaptive aids when necessary.
  • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure coordinated patient care.
  • Participate in case conferences, in-service trainings, and team meetings as required.
  • Educate patients, families, caregivers, and healthcare staff regarding treatment plans and home programs.
  • Complete and submit clinical documentation in a timely manner according to agency standards.
  • Maintain clinical competency and stay current with best practices in speech-language pathology.
  • Identify patient and caregiver needs and make referrals to appropriate services when needed.
  • Supervise Home Health Aides as applicable to the patient's speech therapy plan of care.
  • Perform other related duties as assigned.
