Job summary

Lafayette Nursing and Rehabilitation Center in Mayo, FL seeks a licensed Speech Therapist to assess and treat residents requiring rehabilitation services and to develop individualized goals and treatment plans. The role includes direct patient care, documentation, and patient/family education within a long-term care setting.

Responsibilities include evaluations, progress notes, recertifications, discharge documentation, and collaboration with all disciplines to provide patient-centered care

Qualifications

• Graduated from an accredited Speech Therapist program.
• Must hold a valid professional license and registration.
• Experience in long-term care and/or geriatric care is preferred.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care to evaluate medical condition, functional capabilities, limitations and rehab potential.
• Establish and administer treatment programs with goals per patient capacity under physician direction.
• Set realistic, achievable goals for patients.
• Document and record patient condition.
• Educate patients and families in therapy methods.
• Perform evaluations, weekly progress notes, recertifications and discharge documents.
• Collaborate with all disciplines for patient-centered care.
• Perform other duties assigned by the Director of Rehabilitation.
• Ensure confidentiality, HIPAA and residents' rights.

Duties and Responsibilities:

• Assessment of referred patients that require rehabilitation services


• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.


• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.


• Set realistic and achievable goals for their patients


• Document and record the patients’ condition


• Educate patients and families in an appropriate therapy method.


• Evaluations, weekly progress notes, recertification and discharge documentations.


• Collaborates with all disciplines to provide best patient centered treatment and care.


• Perform all other duties assigned by Director of Rehabilitation


• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements:

• Must have graduated from an accredited Speech Therapist college or university.


• Must have a valid professional license and registration for state of NY


• Experience with long-term care and/or geriatric care is preferred but not limited to.
