Job summary

Topaz Fiscal Services LLC is seeking a licensed Speech Therapist in Florida to assess and treat patients requiring rehabilitation services. You will deliver direct care, establish treatment goals, document progress, and educate patients and families under physician direction.

Responsibilities include completing evaluations, progress notes, recertifications, and discharge documentation while collaborating with a multidisciplinary team to provide patient-centered care, upholding confidentiality

Qualifications

  • Must have graduated from an accredited Speech Therapy program.
  • Valid professional license and registration for NY is required.
  • Experience with long-term care and/or geriatric care is preferred.

Responsibilities

  • Assess referred patients to determine rehabilitation needs.
  • Provide direct patient care to evaluate medical condition and functional status.
  • Develop and manage a treatment plan under physician direction.
  • Set realistic goals tailored to patient capacity.
  • Document patient progress and condition updates.
  • Educate patients and families on therapy methods.
  • Complete evaluations, progress notes, recertifications and discharge documents.
  • Collaborate with multidisciplinary team to deliver patient-centered care.
  • Perform duties assigned by the Director of Rehabilitation.
  • Maintain confidentiality and HIPAA compliance.

  • Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
  • Set realistic and achievable goals for their patients
  • Document and record the patients’ condition
  • Educate patients and families in an appropriate therapy method.
  • Evaluations, weekly progress notes, recertification and discharge documentations.
  • Collaborates with all disciplines to provide best patient centered treatment and care.
  • Perform all other duties assigned by Director of Rehabilitation
  • Compliance with confidentiality, HIPPA and Federal Residents Rights.
