Job summary

Leroy Village Green is seeking a Full-Time Speech Language Pathologist (SLP) to join our resident-centered rehabilitation team.

Part-time and PRN positions are also available, with flexible scheduling and supportive supervision.

You will assess referred patients, develop treatment plans, educate patients and families, document progress, and collaborate with the care team to deliver high-quality, compassionate care.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.

Responsibilities

• Assess referred patients that require rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and rehabilitation potential.
• Establish and administer a treatment program with goals determined by patient capacity and physician direction.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition.
• Educate patients and families in appropriate therapy methods.
• Perform evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborate with all disciplines to provide best patient-centered care.
• Perform other duties assigned by Director of Rehabilitation.
• Ensure compliance with confidentiality, HIPAA and Federal Residents Rights.

Duties and Responsibilities of Speech Language Pathologist (SLP):

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
