Job summary

Greene Meadows Nursing is seeking a PRN Speech Language Pathologist (SLP) to provide assessment and direct patient care in our geriatrics-focused facilities in New York. You will develop and administer treatment plans, document progress, and collaborate with a multidisciplinary team to optimize patient outcomes.

Qualifications

• Must have graduated from an accredited Speech-Language Pathology program.
• Must hold a valid NY state license and licensure to practice.
• Experience in long-term/geriatric care preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation.
• Provide direct patient care and set treatment goals.
• Document patient progress and discharge planning.
• Educate patients and families on therapy methods.
• Collaborate with all disciplines for patient-centered care.
• Maintain HIPAA confidentiality and compliance.

Duties and Responsibilities of Speech Language Pathologist (SLP):

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients’ condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Qualifications/Requirements of Speech Language Pathologist (SLP):

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
