Job summary

Leroy Village Green is seeking a Speech Language Pathologist (SLP) for full-time (PT and PRN also available) to join a resident-centered team focused on quality care. You will assess patients, create and manage treatment plans, document progress, and educate families while collaborating with other disciplines to deliver the best patient-centered rehabilitation.

Applicants should have an accredited SLP degree and active license.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred but not required.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and evaluate medical condition and rehab potential.
• Establish and administer a treatment program with goals aligned to patient capacity.
• Set realistic and achievable goals for patients.
• Document and record the patient’s condition and progress.
• Educate patients and families in therapy methods.
• Complete evaluations, progress notes, recertifications and discharge documents.
• Collaborate with disciplines to provide patient-centered care.
• Perform other duties as directed by the Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.

Duties and Responsibilities of Speech Language Pathologist (SLP):

• Assessment of referred patients that require rehabilitation services
• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
• Set realistic and achievable goals for their patients
• Document and record the patients' condition
• Educate patients and families in an appropriate therapy method.
• Evaluations, weekly progress notes, recertification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
