Job summary

Angels Care Home Health is seeking a Speech Language Pathologist (ST) to provide therapeutic techniques for rehabilitation of patients with speech, language, hearing, swallowing and cognitive disorders in patients’ homes. You will deliver coordinated, evidence-based care and collaborate with an interdisciplinary team.

Requirements include a Master’s degree in Speech Language Pathology, current state licensure, CCC-SLP, a valid driver’s license and 2 years’ experience in a relevant setting.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from ASHA (CCC-SLP).
• Valid driver’s license and reliable transportation.
• 2 years’ experience in acute care, rehab or home health preferred.

Responsibilities

• Provides coordinated care to patients in their home setting.
• Plans, implements and evaluates care plans to restore or maintain well-being.
• Collaborates with interdisciplinary team to optimize outcomes.

The Speech Language Pathologist is a registered professional who provides therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders.

Functions:

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Collaborates with interdisciplinary team.

Qualifications:

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association.
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting, home health preferred.
• Reliable transportation with current auto liability insurance.
