Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide comprehensive care to patients in their homes. The role emphasizes coordination, assessment, and treatment planning as part of an interdisciplinary team.

Qualified candidates hold a Master’s in Speech Language Pathology, current state licensure, and ASHA CCC-SLP certification; a valid driver’s license and 2 years of relevant experience are preferred, with home health exposure advantageous.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited college or university.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from American Speech Language Hearing Association.
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting, home health preferred, reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well being.
• Collaborates with interdisciplinary team.

Education

Master’s degree in Speech Language Pathology

State license in Speech Language Pathology

Certificate of Clinical Competence (CCC-SLP) from ASHA

Valid driver’s license
