Job summary

Angels Care Home Health is seeking a Speech Language Pathologist (ST) to deliver therapy in patients’ homes with a focus on coordinated care and collaborating with an interdisciplinary team.

This full-time role includes a Monday–Friday schedule, a salary plus vehicle allowance and on-call pay, comprehensive health, dental, and vision benefits, generous PTO, CEU access, company-matched 401(k), and opportunities for career advancement.

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Current state license in Speech Language Pathology.
• CCC from American Speech Language Hearing Association (ASHA).
• Valid driver’s license.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.
• Reliable transportation with current auto liability insurance.

Responsibilities

• Provides coordinated care to patients in their residence.
• Plans, implements, and evaluates patient care plans to restore or maintain well-being.
• Collaborates with interdisciplinary team.

Education

Master’s degree in Speech Language Pathology

Current state license in Speech Language Pathology

Certificate of Clinical Competence (CCC)

Valid driver’s license

2 years’ experience in acute care or rehabilitation

Reliable transportation with auto liability insurance

The Speech Language Pathologist is a registered professional who provides therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing, and cognitive disorders.

Functions:

• Provides coordinated care to patients in their place of residence.
