Job summary

Aventura at Creekside is actively seeking a PRN Speech-Language Pathologist (SLP) to join our compassionate in-house rehabilitation team in Carbondale, PA. This role supports residents with communication, cognitive, and swallowing needs and offers flexible scheduling.

Qualifications

• Must be a licensed or eligible Speech-Language Pathologist in the state where employed
• Experience with hospital or rehabilitation settings is a plus

Responsibilities

• Provide speech-language pathology services to residents and patients experiencing communication, cognitive, and swallowing disorders
• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function
• Deliver treatment in accordance with the established plan of care
• Educate and support patients, families, and caregivers on therapy programs and progress
• Monitor and report patient responses to treatment interventions
• Maintain accurate and timely documentation in compliance with company policies and state/federal regulations
