Job summary

Aventura at Creekside in Carbondale, PA is seeking a PRN Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. New grads welcome to apply as part of a compassionate caregiving environment that prioritizes patient-centered care and team collaboration.

Responsibilities include evaluating residents, developing individualized treatment plans, delivering therapy, and educating families and caregivers on progress and programs.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Current licensure/certification as a Speech-Language Pathologist.
• Experience with residents/patients who have communication, cognitive, and swallowing disorders preferred.

Responsibilities

• Provide speech-language pathology services to residents/patients with communication, cognitive, and swallowing disorders.
• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function.
• Deliver treatment in accordance with the established plan of care.

Key Responsibilities

• Provide speech-language pathology services to residents and patients experiencing communication, cognitive, and swallowing disorders
• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function
• Deliver treatment in accordance with the established plan of care
• Educate and support patients, families, and caregivers on therapy programs and progress
• Monitor and report patient responses to treatment interventions
• Maintain accurate and timely documentation in compliance with company policies and state/federal regulations
