Job summary

Aventura at Creekside is actively seeking a PRN Speech-Language Pathologist (SLP) to join our in-house rehabilitation team. The role focuses on helping residents improve communication, swallowing, and quality of life in a collaborative setting.

You will work within an interdisciplinary team, develop individualized plans of care, and provide education to patients and families, while ensuring compliant documentation and ongoing support.

Qualifications

• Master's degree in Speech-Language Pathology required or equivalent.
• Licensed or eligible for licensure in the state where practicing.
• Experience in hospital, SNF, or rehabilitation settings is preferred.

Responsibilities

• Provide speech-language pathology services to residents and patients with communication, cognitive, and swallowing disorders.
• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function.
• Deliver therapy according to the established plan of care and state regulations.
• Educate patients and families on therapy programs and progress.
• Monitor and report patient responses to treatment interventions.
• Maintain accurate documentation in compliance with policies and regulations.

Key Responsibilities

• Provide speech-language pathology services to residents and patients experiencing communication, cognitive, and swallowing disorders

• Evaluate patients and develop individualized treatment plans to improve communication, cognition, swallowing, and overall function

• Deliver treatment in accordance with the established plan of care

• Educate and support patients, families, and caregivers on therapy programs and progress

• Monitor and report patient responses to treatment interventions

• Maintain accurate and timely documentation in compliance with company policies and state/federal regulations
