  • High level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service.

Job description

  • Coordinate between physician offices, specialty clinics, hospital departments, and patients for clinic appointments, procedures, and referrals
  • Serve as an essential link between patients and care providers
  • Maintain assigned patient cases longitudinally
  • Track and maintain patient profiles, appointments, interactions, and other data
  • Coordinate multidisciplinary planning conferences
  • Develop concise patient summaries and document care-team recommendations using standardized protocols and national care guidelines
  • Assess gaps related to patient outcomes and support continuous quality improvement
  • Compile and share statistics and data with management
  • Contact patients before appointments to confirm attendance and identify barriers
  • Obtain, record, and retrieve patient information using the Medical Information system
  • Prepare the care team for patient arrival
  • Enter test orders and insurance authorizations where applicable
  • Mail specimen/sample kits to patients where applicable
  • Identify medical, insurance, financial, and social resources and provide community referrals
  • Follow up with patients and families throughout care plans
  • Escort patients and families to pre- and post-procedure areas when needed
  • Identify opportunities to improve patient care and organizational processes
  • Resolve customer problems and anticipate internal and external customer needs
  • Contribute to hospital, clinic, and patient-care-team performance improvement
  • Use job-related software and systems
  • Complete required trainings, submit invoices, and perform other duties according to standard operating procedures
  • Assist eligible clients with applications, supporting documentation, and agency submission
  • Provide referrals to and collaborate with onsite Case Managers
  • Schedule and complete patient appointments and provide holistic support for self-management and medical stability
  • Work collaboratively as part of a Ryan White Service team
  • Provide information, resources, and support to help patients remain in care, adhere to medication, and maintain health and well-being
  • Help patients access services from multiple providers and bridge communication between patients and providers
  • Coordinate non-emergency medical transportation through the comprehensive Lyft program
  • Assist with linkage and navigation of HIV PrEP care
Requirements
  • High School diploma or equivalent; one year of customer service or medical experience; or equivalent combination of education and experience
  • Familiar with guidelines regarding patient confidentiality issues
  • Working knowledge of MS Office platforms such as Excel, OneNote, Word, and PowerPoint
  • Ability to convey information both in writing and verbally
  • High level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service
  • Sensitivity to, interest in and competence in cultural differences, HIV/AIDS, sexual practices, and substance dependency
  • Demonstrated competence in working with the gay/lesbian/bisexual/transgender community
