* Work with Patients to schedule and complete appointments, provide holistic support to increase patient self-management, and assist in achieving medical stability
* Work callobratively as part of a Ryan White Service team.
* Provide Patients wiht the information, resources, and support they need to remain in care, adhere to their medication, and maintain good health ans well-being
* Help Patients to access servcies from multiple providers, acting as a bridge between patients and providers
* Coordinate scheduling of non emergency medical transportation via comprehensive Lyft program
* Assist with linkage and navigation of HIV Pre-exposure Prophylaxis (PrEP) Care
High School diploma or equivalent; one year of customer service or medical experience; or equivalent combination of education and experience. Familiar with guidelines regarding patient confidentiality issues. Must have working knowledge of MS Office platforms such as (Excel, OneNote, Word, PowerPoint). Ability to convey information both in writing and verbally. Must have high level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service.
* Must have sensitivity to, interest in and competence in cultural differences, HIV/AIDS, sexual practices, substance dependency and a demonstrated competence inworking with the gay/lesbian/bisexual,transgender community.
