Job summary

STRIDE is seeking a patient care coordinator to work with the care team to coordinate services for patients. You will use evidence-based screening tools to identify social determinants of health and health-related needs, and document findings.

You will provide resources and referrals, maintain a database of community resources, and communicate with patients and the care team throughout the care process. The role requires compassion, confidentiality, and strong communication.

Qualifications

  • High school diploma or GED is required.
  • Active BLS certification is required.
  • 1 year of direct or indirect support of patient care or related experience is preferred.
  • 1 year of experience in a community health or Federally Qualified Health Center setting is preferred.
  • Additional language proficiency highly desired.
  • Ability to interact positively and build rapport with patients, coworkers and external contacts.
  • Ability to respond to the needs and concerns of STRIDE’s diverse patient population effectively and sensitively.
  • Ability to handle sensitive information ethically and responsibly.
  • Ability to protect the confidentiality of patient, employee, and business information.
  • Ability to discern information from others in a variety of formats and communicate information clearly.
  • Ability to work independently while ensuring accuracy and efficiency.
  • Ability to demonstrate empathy with cultural and diversity dynamics.
  • Advanced customer service skills, including ability to diffuse upset patients.
  • COVID-19 vaccination
  • Annual influenza vaccination, or approved exemption
  • Tuberculosis screening and testing
  • Ability to pass background checks, including criminal history, education, license and certification

Responsibilities

  • Coordinate internal and external services for patients in collaboration with the care team
  • Use evidence-based screening tools to identify and document social determinants of health and health-related social needs
  • Provide resources and referrals to internal and external partners addressing identified barriers
  • Maintain the database of community resources available to STRIDE’s patient population
  • Document and communicate with patients and the care team from initial interaction through closure or completion of goals
  • Conduct respectful, collaborative, and confidential patient interactions using motivational interviewing principles when appropriate
  • Provide basic health education using evidence-based resources and refer patients for further education and support
  • Manage internal and external referrals, including processing, follow-up, database maintenance, report retrieval, and indexing
  • Contribute to population health efforts through targeted patient outreach and scheduling
  • Follow departmental workflows and support quality improvement activities
  • Complete other duties as assigned
