Job summary

Sentara Healthcare in Chesapeake, VA, seeks a Pharmacy Navigator to guide specialty patients through their medication journey. You will coordinate refills, address insurance challenges, and work with pharmacists, prescribers, and clinic staff to ensure seamless patient support.

Role requires a Pharmacy Technician license and at least 2 years of experience. Excellent communication and attention to detail help ensure timely deliveries and adherence across care teams.

Qualifications

• Pharmacy technician license valid in state(s) of practice.
• At least 2 years of pharmacy tech experience.
• Ability to coordinate refills, insurance PA handling, and patient support.

Responsibilities

• Deliver exceptional, personalized care to specialty pharmacy patients, providing a full suite of medication management services.
• Collaborate with clinic and ambulatory care teams, supporting patients in person and over the phone.
• Educate patients on the benefits of the preferred specialty pharmacy and its impact on care outcomes.
• Support outpatient retail and specialty prescription needs, including refills and adherence.
• Monitor patients’ regimens through outbound check-ins and timely refills.
• Identify medications requiring special handling or storage, such as hazardous or refrigerated drugs.
• Respond to, resolve, and triage inbound patient and provider inquiries.
• Address insurance-related issues, including prior authorizations, and assist with financial assistance programs.
• Maintain accurate documentation in required systems and tracking mechanisms.
• Manage and organize patient and operational data using relevant database applications.
• Apply insights from patient interactions to improve workflow, productivity, and service quality.
• Perform other duties as assigned.
