Job summary

Sentara Health Plans is seeking a Pharmacy Navigator to join our team in Chesapeake, VA. This hybrid, full-time role guides specialty pharmacy patients through their medication journey, coordinating refills and deliveries while addressing insurance or financial challenges.

You will work closely with pharmacists, prescribers, and clinic staff to resolve patient concerns and ensure timely, personalized care. A valid Pharmacy Technician license and two years of experience are required.

Qualifications

• Pharmacy Technician license valid in state(s) of practice required.
• 2 years of pharmacy technician experience required.

Responsibilities

• Guide specialty pharmacy patients through their medication journey and coordinate refills and deliveries.
• Monitor patient adherence and help resolve insurance or financial challenges.
• Collaborate with pharmacists, prescribers, and clinic staff to address patient concerns quickly.
• Educate patients on the benefits of the preferred specialty pharmacy and its impact on care outcomes.
• Support outpatient retail and specialty prescription needs, including managing refills.
• Maintain accurate documentation and organize patient and operational data in required systems.
