Job summary

Sentara Health is hiring a Pharmacy Navigator in Chesapeake, VA to guide specialty pharmacy patients through their medication journey. You will coordinate refills and deliveries, monitor adherence, and help resolve insurance or financial challenges while collaborating with pharmacists, prescribers and clinic staff.

Deliver exceptional, personalized care to patients, educate them on the benefits of the specialty pharmacy, and support outpatient prescription needs.

Qualifications

• 2 years of pharmacy tech experience required.
• Pharmacy Technician license valid in state(s) of practice required.

Responsibilities

• Guide specialty pharmacy patients through every step of their medication journey.
• Coordinate refills and deliveries, monitor adherence, and resolve insurance or financial challenges.
• Educate patients on the benefits of the preferred specialty pharmacy and its impact on care outcomes.
• Support outpatient retail and specialty prescription needs, including managing refills and ensuring medication adherence.
