Qualifications

ASHP-Accredited PGY-1 residency or 3 years of clinical pharmacy experience preferred.
Active Tennessee license required by start date; partial remote role in Ascension Tennessee market.

Responsibilities

Provide advanced clinical pharmacy care with counseling and pharmacotherapy management for complex specialty patients.
Conduct comprehensive MTM to identify drug interactions and optimize outcomes.
Develop and monitor individualized care plans, track labs, clinical progress, and adverse events.
Provide evidence-based recommendations on drug therapy, dosing, and safety to providers.
Document in EHR and specialty pharmacy software to meet accreditation standards.
Collaborate with interprofessional teams to streamline prior authorizations, logistics, and transitions.
