Job summary

Ascension is seeking a pharmacist to provide advanced clinical pharmacy care, including counseling and ongoing pharmacotherapy management for patients with complex specialty conditions. You will perform MTM, develop care plans, monitor labs, and collaborate with providers to optimize therapy and safety.

This remote role supports Infectious Disease (HIV) clinics servicing the Indianapolis area. Indiana license active by start date is required; ASHP PGY residencies or equivalent experience

Qualifications

• Pharmacist license required in Indiana by start date.
• Pharmacy degree required; PharmD preferred.
• ASHP-Accredited PGY-1 residency or 3 years of relevant clinical experience preferred.
• Indiana license active by start date; remote role for Infectious Disease (HIV) clinics in the Indianapolis area.

Responsibilities

• Provide advanced clinical pharmacy care including counseling and ongoing pharmacotherapy management for patients with complex specialty conditions.
• Perform comprehensive medication therapy management (MTM) to identify drug interactions and optimize therapeutic outcomes.
• Develop and monitor individualized care plans, tracking laboratory values, clinical progression, and adverse drug events.
• Provide evidence-based clinical recommendations to healthcare providers regarding drug selection, dosing adjustments, and patient safety strategies.
• Document in the electronic health record (EHR) and specialty pharmacy software to meet accreditation standards.
• Collaborate across interprofessional teams to streamline prior authorizations, medication delivery logistics, and care transitions.
