Job summary

Clearway Health is seeking an MTM Clinical Pharmacist to optimize patient outcomes for chronic and specialty medications. You will review therapies, resolve medication-related issues, and coordinate care with physicians, nurses, and payors, while educating pharmacists, residents, and students.

You will lead or support initiatives to improve workflow, adhere to URAC/ACHC standards, and contribute to formulary strategies.

Qualifications

PharmD or B.Pharm required.
PGY-1 residency and 3+ years direct patient care experience required.
PGY-2 ambulatory care or specialty residency preferred.

Responsibilities

Conduct comprehensive medication therapy reviews for chronic/specialty patients.
Provide disease-specific counseling and education to patients and staff.
Collaborate with prescribers, nurses, case managers, and payors.
Support prior authorization, appeals, and benefits verification processes.
Monitor clinical outcomes, adverse events, and adherence metrics.
Document and report interventions to support quality improvement.
Ensure adherence to URAC/ACHC standards and payor requirements.
Maintain documentation for audits and accreditation readiness.
Contribute to developing clinical protocols and formulary strategies.
Lead or support workflow optimization and patient satisfaction initiatives.
