Covenant Health’s Fort Sanders Regional Medical Center in Knoxville, TN is hiring a Pharmacist for a full-time, day-shift position. The Pharmacotherapy Clinical Pharmacist will deliver comprehensive pharmaceutical care, optimize medication therapy, and partner with clinicians to improve patient outcomes.

The role emphasizes MTM, chronic disease management, patient education, and collaboration across care teams, with ongoing quality improvement and education responsibilities.

Qualifications

• Licensed pharmacist in Tennessee; PGY-1 Residency or 2+ years of ambulatory care experience preferred.
• 1–3 years of experience in a clinical setting.
• Current state pharmacy license.

Responsibilities

• Medication Therapy Management (MTM): conduct comprehensive reviews for chronic conditions and provide initial, ongoing counseling.
• Collaborative Practice: develop and implement care plans with physicians and nurses.
• Patient Education: educate patients and caregivers on medication use, interactions, and lifestyle changes.
• Chronic Disease Management: monitor and adjust regimens per guidelines and patient progress.
• Adherence Monitoring: track adherence, identify barriers, and refer to financial counseling as needed.
• Clinical Documentation: maintain detailed patient records of interventions and outcomes.
• Consultation Services: advise on drug selection, dosage, and management of side effects.
• Laboratory Monitoring: review labs to inform therapy decisions.
• Quality Improvement: participate in clinic-based projects to improve outcomes.
• Continuing Education & Training: stay current with guidelines and provide education to team.
