Job summary

Sentara Norfolk General Hospital in Norfolk, VA is seeking a Specialty Clinical Pharmacist to join the Case Management team. The role involves managing patient drug therapy, reviewing orders for safety, and providing detailed drug information and counseling in a rotating day schedule.

The position requires a Bachelor’s in Pharmacy, Virginia Pharmacist License, and BLS (ACLS preferred). Clinical experience with specialty medications and patient education are highly valued.

Qualifications

  • Bachelor's Degree in Pharmacy is required.
  • Doctorate in Pharmacy is preferred.
  • Virginia Pharmacist License is required.
  • BLS is required; ACLS is preferred.
  • Previous Clinical Experience is preferred.
  • Drug Counseling Experience and knowledge of Specialty Medications.

Responsibilities

  • Provide safe, effective, and prompt pharmacy services.
  • Manage patient drug therapy per standards of practice.
  • Review provider medication orders for appropriateness and safety.
  • Serve as primary drug information resource and educate patients as needed.
  • Counsel patients on medications and therapy options.
  • Participate in training of new employees and maintain local ownership of tasks.

City/State Norfolk, VA Work Shift Rotating Overview: Sentara is hiring a Specialty Clinical Pharmacist for our Case Management team at Norfolk General Hospital

Location: Norfolk, VA

Status: Full Time (40 hrs/wk)

Shift: Rotating Days shift

Hours: 8am-4:30pm / 8:30am-5pm / 9:30am-6pm Occasional Saturdays

The Specialty Clinical Pharmacist is responsible and accountable for the provision of safe, effective, and prompt pharmacy services.

Participates in all necessary aspects of the medication-use system while providing comprehensive and individualized pharmaceutical care to the patients in their assigned areas.

Demonstrates the ability to work in areas of pharmacy practice competently and independently

Manages patient drug therapy in accordance with applicable clinical practice standards

Assures the appropriateness of all provider medication orders in maintaining a high level of patient care and safety

Maintains current knowledge of medication use and disease states applicable to their area of practice

Serves as the primary resource for drug information and education

Provides in-depth patient counseling when required

Participates in the training of new employees as assigned

Assumes local responsibility and ownership for assigned tasks

Consistently follows policies and procedures

Continually seeks out additional training opportunities and stays abreast of new and changing information for assigned areas

Demonstrates superior customer service, effective communication, decision-making, and organization to ensure efficient job performance and success

Serves as a role model in the following areas: professionalism, collaboration, and teamwork.

Ensures adequate supplies are available for continuous patient care

Triages and escalates any questions or issues outside the scope of practice as needed

Uses appropriate systems for processing of patient information

Ensures quality assurance is maintained according to departmental and legal requirements

Operates pharmacy equipment appropriately

Precepts pharmacy students as assigned

In addition to the above job responsibilities, other duties may be assigned

Education

Bachelors Level Degree in Pharmacy (Required)

Doctorate Level Degree in Pharmacy (Preferred)

Certification/Licensure

Virginia Pharmacist License (Required)

Basic Life Support (BLS) (Required)

Advanced Cardiac Life Support (ACLS) (Preferred)

Experience

Previous Clinical Experience Preferred

Drug Counseling Experience

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $148,304.00 - $214,926.40 annually.

Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full-time colleague working 2,080 hours annually.
