Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in the community and in-office settings for patients and their families.

The Case Manager will assist the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress, and help gain access to health services, financial assistance, employment, education, social services, and natural supports.

Qualifications

  • 1–3 years of experience working with GMH/SU population.
  • 1–3 years of outreach and engagement in behavioral health.
  • 1–3 years of experience in a medical setting preferred.
  • Minimum of six months of recovery from substance use or mental health disorders preferred.

Responsibilities

  • Provide case management in the community and/or in-office settings.

  • Help patients access health services, financial assistance, employment, education, social services, and natural supports.

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona, including residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary

The Case Manager provides case management services in the community and/or in‑office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services, and natural support. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.

Qualifications
  • High school diploma or GED required. Associate’s degree or higher in a field related to behavioral health preferred.
  • 1–3 years of experience working with the GMH/SU population required.
  • 1–3 years of experience in a behavioral health position providing outreach and engagement activities required.
  • 1–3 years of experience in a medical setting preferred.
  • Minimum of six months of recovery from substance use and/or mental health disorders preferred.
  • Current AZ Driver’s License (valid and in good standing), preferred.
  • Valid ID required.
  • Clean 39‑month driving record (MVR) preferred.
  • Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures.
  • Arizona Fingerprint Clearance card.
