Community Bridges, Inc. (CBI) is seeking a Case Manager to provide case management services in the community and in-office settings for patients and their families.
The Case Manager will assist the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress, and help gain access to health services, financial assistance, employment, education, social services, and natural supports.

The Case Manager provides case management services in the community and/or in‑office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services, and natural support. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and services provided in an accurate and timely manner.
