Job summary

Qualifications

• High School diploma or equivalent; one year of customer service or medical experience or equivalent combination of education and experience.
• Familiar with guidelines regarding patient confidentiality issues.
• MS Office platforms such as Excel, OneNote, Word, and PowerPoint.
• Ability to convey information both in writing and verbally.
• High level of interpersonal skills to handle sensitive and confidential situations while providing exceptional customer service.

Responsibilities

• Coordinate between physician offices, specialty clinics, hospital departments, and patients for clinic appointments, procedures, and referrals.
• Serve as essential link between patients and care providers.
• Maintain assigned patient cases longitudinally.
• Track and maintain patient profiles, appointments, interactions, and other data.
• Coordinate multidisciplinary planning conferences.
