Job summary

Qualifications

• Graduate of a master's level nurse practitioner program OR from an accredited physician assistant program.
• 1–3 years work experience in a clinical setting preferred.

Responsibilities

• Provides assessment, evaluation, and management to patients including minor, acute, chronic, and episodic illness and injuries.
• Collaborates with patients to review health background, provide counseling and education for health plans and prevention.
• Performs follow-up services, orders appropriate diagnostic tests, may prescribe medications and non-pharmacological treatments.
• Maintains and reviews patient records and documents in the EMR.
• Collaborates with physicians and other providers to develop and implement patient care plans.
• Provides case management and referrals to needed services.
