Job summary

Prevea Health seeks an Advanced Practice Provider (NP or PA) to deliver high-quality care to patients, families and groups in Wisconsin. This 32-hour-per-week role emphasizes health promotion and disease prevention under physician supervision.

You will assess, diagnose, and manage a range of conditions, collaborate with physicians, and coordinate care through a strong patient-centered approach and robust documentation in the EMR.

Qualifications

• Graduate of a master's level NP program or graduate of an accredited PA program; required.

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, and episodic illness and injuries, including follow-up exams.
• Collaborates with patients to review health background and plan future health actions.
• Provides case management to arrange or refer needed services.
• Conducts telephone follow-ups and provides health counseling and education.
• Orders diagnostic tests, prescribes medications, and other non-pharmacological treatments.
• Maintains and reviews patient records in the EMR.
• Collaborates with physicians to develop and implement care plans.
