• Collaborates with patients to review health needs and plan prevention strategies.
• Provides case management services and coordinates referrals as needed.
• Performs follow‑up calls and communicates test results to patients.
• Orders diagnostic tests and prescribes medications as appropriate.
• Maintains patient records in the EMR system with accurate documentation.
• Collaborates with physicians to develop and implement care plans.
