Qualifications

• Graduate of a master's level NP program or graduate from an accredited PA program.
• 1-3 years work experience in a clinical setting preferred.

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries.
• Collaborates with patients to review health background and plan future health strategies.
• Provides case management services to arrange or refer patients to needed services.
• Conducts follow-up calls and provides health counseling and follow-up instructions.
• Orders diagnostic tests, prescribes medications, and documents in EMR.
• Maintains and reviews patient records in the EMR.
• Collaborates with physicians to develop and implement patient care plans.
