Job summary

Prevea Health is seeking an Advanced Practice Provider (NP or PA) to deliver health care services under physician supervision. The role emphasizes health promotion and disease prevention with patients, families, and groups in a collaborative care setting in Wisconsin.

The provider will perform exams, manage cases, review patient histories, coordinate care, and contribute to program development while maintaining EMR documentation and following clinical guidelines.

Qualifications

• Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program.
• Wisconsin RN license or compact equivalent with NP certification, or Wisconsin PA license with NCCPA, Upon Hire.
• CPR/Basic Life support certification required.

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, and episodic illness and injuries.
• Collaborates with patients to review health background and advise on future health plans and prevention.
• Provides case management services to arrange or refer needed services.
• Conducts telephone follow-up and provides health counseling and education.
• Orders diagnostic and laboratory tests and prescribes medications when appropriate.
• Maintains patient records in the electronic medical record.
• Collaborate with physicians to develop and implement patient care plans.
