Qualifications

Responsibilities

• Provides assessment, evaluation, and management of minor to acute illnesses and injuries.
• Collaborates with patients to review health history and plan preventive care.
• Provides case management and coordinates referrals as needed.
• Makes follow-up calls, provides health counseling and follow-up instructions.
• Orders appropriate tests, prescribes medications, and documents in EMR.
• Develops and implements patient care plans with physicians.
