Job summary

Qualifications

• Bachelor's degree in education is required.
• TOSD certification is preferred but not required at this time.

Responsibilities

• Facilitates transition programming for students eligible for special education and with IEP transition goals.
• Acts as liaison with local Transition Coordinating Council.
• Collaborates with high school and middle school admins, teachers, and counselors to facilitate transition programming.
• Networks with community agencies and organizations providing post-secondary services.
• Attends IEP meetings in the role of Transition Teacher.
• Ensures IEP teams develop and implement transition outcomes and services.
• Coordinates school and community work-based learning opportunities.
• Communicates with parents, students, staff and providers about transition issues.
