Job summary

New London Public Schools seeks a Special Education Transition Coordinator to lead district-wide transition services for students with disabilities as they plan for life after high school. You will collaborate with families, school staff, and community agencies to develop IEP goals, coordinate employment and independent living experiences, and ensure access to meaningful transition opportunities.

Requires CT CSDE certification in Comprehensive Special Education and several years of secondary

Qualifications

Must hold a CT CSDE educator certificate with an endorsement in Comprehensive Special Education (065,165, or 265).
Three or more years of experience in Secondary Special Education and/or Vocational Transition Education/Services.
Demonstrated experience in writing standards-based IEPs with transition goals.
Knowledge of Connecticut Core Transition Skills and CT Standards for Transition Planning.
Experience developing programming for transition planning in post-secondary education, employment, and independent living.

Responsibilities

Conduct vocational and transition assessments and develop person-centered post-secondary goals.
Coordinate transition services, community-based learning, and employment opportunities aligned with IEPs.
Build partnerships with families, employers, and community organizations to facilitate referrals and resources.
Develop and implement transition programming including career readiness, self-determination, and transportation training; support job coaches.
