Job summary

iCare Health Network in Hartford, Connecticut is seeking a full-time Social Worker, preferably bilingual in Spanish. The Social Worker coordinates and supervises the delivery of essential social services to residents while adhering to professional standards and regulations.

The ideal candidate will have a Bachelor’s Degree in Social Work and at least one year of experience in long-term care. This position offers a collaborative work environment and includes benefits such as health insurance and a 401(k) retirement plan.

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• Minimum of one year’s experience in long-term care preferred.
• Spanish speaking preferred.

Responsibilities

• Conduct resident interviews and psychosocial assessments.
• Participate in planning for resident discharges.
• Respond to clinical emergencies and regulatory agency visits.
• Direct and supervise delivery of social services to residents.
• Collaborate with interdisciplinary treatment team for community referrals.
• Participate in developing new psycho-social service programs.
