Job summary

iCare Health Network in Hartford, CT is seeking a full-time Social Worker to coordinate and supervise social work services for residents, ensuring compliance with state and federal regulations.

Qualifications

• Bachelor’s Degree in Social Work or related human services field is required or equivalent.
• A minimum of one year’s experience in long-term care providing direct social services is preferred. Spanish-speaking ability is preferred.

Responsibilities

• Conduct face-to-face resident interviews and assessments of cognitive, mood, behavioral status and other psychosocial factors.
• Plan for resident discharges.
• Respond to clinical emergencies, regulatory visits, surveys, and issues outside normal work hours as needed.
• Direct and supervise the delivery of medically related social services to residents and their families.
• Participate in clinical evaluation and screening of potential admissions.
• Collaborate with interdisciplinary team to refer residents to community services as needed.
• Serve as liaison with mental health practitioners and consultants.
• Develop and implement new behavioral and psychosocial programs and present them to physicians, families, and residents.
