The Phoenix Rehabilitation and Nursing Center is seeking a compassionate and motivated bilingual Social Worker (Spanish/English) to join our interdisciplinary care team in a skilled nursing setting. Qualifications

• Bilingual in Spanish and English required.
• Healthcare or long-term care experience preferred.
• Excellent communication and organizational skills.
• Knowledge of CMS/state regulations.

Responsibilities

• Conduct psychosocial assessments and develop care plans.
• Provide counseling and emotional support to residents and families.
• Serve as liaison between residents, families, and interdisciplinary team.
• Facilitate discharge planning and coordinate community resources.
• Participate in admissions and resident orientation.
• Maintain accurate documentation in the EMR system.
• Participate in care conferences and family meetings.
• Ensure compliance with CMS and state regulations.
