Job summary

iCare Health Network seeks a full-time Social Worker, preferably Spanish speaking, to coordinate and supervise social work services for residents in a long-term care facility.

The role involves interviews, assessments, discharge planning, and collaboration with an interdisciplinary team to access community-based services and liaise with contracted mental health professionals. Generous benefits and pension are included.

Qualifications

• Bachelor’s Degree in Social Work or related field is required.
• One year experience in long term care providing direct social services preferred.
• Spanish speaking ability preferred.

Responsibilities

• Conduct face to face resident interviews and assess cognitive, mood, behavioral status and other psychosocial factors.
• Plan for resident discharges.
• Respond to clinical emergencies, regulatory visits, surveys and after-hours issues as needed.
• Direct and supervise the delivery of medically related social services to residents and families.
• Collaborate with the facility interdisciplinary treatment team to access community-based services.
• Serve as liaison with contracted mental health practitioners and consultants.
• Develop and implement new psychosocial programs and present them to physicians, families and residents.
