Qualifications

Responsibilities

• Advocate for the well-being of residents and ensure holistic needs are met.
• Conduct comprehensive psychosocial assessments to identify social, emotional, and environmental needs, strengths and preferences.
• Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans.
• Provide counseling and emotional support to residents and families, guiding coping strategies and decision-making processes.
• Advocate for residents’ rights and preferences in care planning and decisions.
• Connect residents and families with community resources and advocacy organizations.
• Provide crisis intervention and support during challenging situations (grief, health changes).
