Spanish Trails Wellness & Rehabilitation is seeking a licensed social worker to identify psychosocial and emotional needs, develop plans, and aid in accessing services for residents. The role adheres to NASW Code of Ethics and applicable regulations, with responsibilities spanning assessment, care planning, documentation, and discharge coordination.

The position emphasizes resident rights, family education, and inter-disciplinary collaboration to enhance quality of life in a long-term care

Qualificações

• Certified, licensed, or registered in the state of practice, required.
• Bachelor’s Degree in Social Work or a Bachelor’s Degree in Human Services field from an accredited school of social work, required.
• One year of supervised social work experience in a healthcare setting with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsabilidades

• Performs social history and psychosocial assessments to identify social, emotional, and psychological needs; uses MDS to guide care planning.
• Maintains accurate documentation per regulations, policies, and care standards.
• Supports resident rights and documents refusals; informs on therapeutic alternatives as appropriate.
• Develops written interdisciplinary care plans addressing psychosocial needs and goals.
• Provides therapeutic interventions to support coping with transition to long-term care.
• Offers family education and referrals to social services when needed.
• Coordinates discharge planning and referrals for home health services prior to community return.
• Collaborates with interdisciplinary team to protect resident rights and well-being.
• Contributes to staff education and ensures compliance with standards during inspections.
• Other duties may be assigned as needed.

Conhecimentos

Licensed in state

BS in Social Work or Human Services

Supervised social work experience

Nursing experience in SNF (preferred)
