Job summary

Cityblock Medical Practice, P.A. is seeking an RN Case Manager to manage a panel of members with chronic disease and behavioral health needs. You will collaborate on care plans, support progress, and coordinate with providers to meet member goals in a hybrid work setting in the Bronx.

The role requires 3+ years of RN experience, NY license, and Spanish proficiency. You will participate in care facilitation, use EHRs, and support scheduling and telehealth activities.

Qualifications

• 3+ years of RN experience.
• Bilingual Spanish.
• Unrestricted RN license in New York.
• Experience with telehealth and virtual visits.
• Experience with scheduling and EHR systems.

Responsibilities

• Engagement with members to describe program expectations and goals.
• Assessments and screenings including PHQ9, GAD-7, AUDIT, DAST-10.
• Coordinate with care team for appropriate program placement.
• Develop care plans with members and collaborate on social needs.
• Regular member check-ins and follow-ups as guided by the program.
