Qualifications

• High School diploma or GED required.
• Experience in acute health care setting preferred.
• Experience in interdisciplinary teams required.
• Ability to read, write and speak English; bilingual in English and Spanish preferred.

Responsabilités

• Initiate communication with patient upon receiving a referral.
• Develop, coordinate and provide services to patients and families.
• Promote the facility mission, vision and values in daily practices.
• Support authorization turnaround time and approval rate efforts.
• Assist with scheduling, patient preparation, and transportation resources as needed.
• Other duties as assigned.
