Qualifications

• Must speak Spanish.
• Must have knowledge of HHA Exchange.
• 1 year of Intake Experience (Preferred).

Responsibilities

• Receive referrals from referral sources.
• Process referrals from marketing sources such as hospitals, nursing homes, and community physicians.
• Verify patients' insurance to determine eligibility.
• Educate patients on services provided and what is approved by insurance.
• Coordinate home care needs for skilled and non-skilled services with patients.
